Notice of Copyright Infringement

If you believe that your copyright-protected work has been uploaded, posted, or otherwise made accessible through our website without your authorization, you may submit a notification of claimed infringement to us. It is important to provide a clear and detailed message so that we can adequately address your concerns. All notifications should comply with the statutory requirements of the DMCA and must include the following information:

  • A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
  • Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
  • Information reasonably sufficient to permit the service provider to contact the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which the complaining party may be contacted.
  • A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Submitting a DMCA Notice

Notices of alleged infringement should be sent directly to our designated agent via email at dmca@texasformspdf.com. We take all valid complaints seriously and will review your claim promptly. Upon receiving a compliant notification, it is our policy to remove or disable access to the infringing material, notify the content provider, member, or user that we have removed or disabled access to the material, and, for repeat offenders, to terminate such content provider's, member's, or user's access to the service.

Counter-Notice Procedures

If you receive a notification that material made available by you on or through the website has been taken down, you may provide a counter-notification in accordance with the requirements of the DMCA. Your counter-notification must include substantially the following:

  • Your physical or electronic signature.
  • Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or access to it was disabled.
  • A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or a misidentification of the material to be removed or disabled.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal district court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial DIRECTORY where the service provider may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.

Please note that there are legal and financial consequences for fraudulent and/or bad faith submissions. Therefore, if you are not certain that material located on or linked to by our website infrits your copyrighted, we strongly encourage seeking legal advice before filing a DMCA notice with us.

Our policy is to respect the intellectual property rights of others and to provide a platform that fosters the sharing of legally compliant information. By working collaboratively with copyright owners and our user community, we aim to maintain a respectful and lawful environment.
